We collect your name, contact details, relationship to the person interested, broad age range, support interests, area and contact preferences.

Please do not include medical, medication, financial, bank-card, password, PIN or key-safe information. If you are completing this form for someone else, only share information you are authorised to provide and keep it to what is necessary for the enquiry.

We use your information because we have a legitimate interest in responding to enquiries and providing the requested information. We do not sell your data or use this form to make automated decisions. We keep an enquiry for up to six months after our last meaningful contact if no service begins.
